If you have higher care needs and are considering accessing a Home Care Package, follow the steps below to begin the process.

1. Check your eligibility

If you have higher care needs and are considering accessing a Home Care Package, you will first need to contact My Aged Care by calling 1800 200 422 or going to www.myagedcare.gov.au.  

To be eligible for a Home Care Package, you must be an older adult who requires various services to help you live independently at home. Your eligibility will be confirmed through an ACAT assessment, identifying whether you have high or low-level care needs. 

Your financial situation won’t impact whether you’re eligible, but it may be assessed to determine the amount you might need to contribute.  

If you are assessed as needing lower-level assistance, a different program called the Commonwealth Home Support Programme or Short-Term Restorative Care could be a better fit.

For Home Care Packages

The Client Engagement Team will connect you with one of our Wellness Partners, who will organise a time to visit. In the meeting, they will discuss the different types of support available to you under your pre-approved package, explain the management fees and put together a sample Home Care Package budget to show you where your funds will be allocated. If everything aligns with your needs and expectations, you can accept the package. If you wish to proceed, you can fill out the necessary paperwork. If not, the team can arrange a follow-up call at another time to check if you would like to move forward.

For the Commonwealth Home Support Programme

The Client Engagement Team will arrange a suitable day and time for a Service Coordinator to visit and complete the necessary paperwork to start the service. The Service Coordinator will get in touch on the agreed day to confirm the meeting time.

4. Begin your services

Your Wellness Partner or Service Coordinator will now work with you to develop a care plan.  

The plan should be tailored to meet your unique needs, preferences and goals so that you can continue living independently in the community for as long as possible.
