We are committed to supporting our clients to live independently at home for as long as possible. For some, home care might mean having a bit of extra help with everyday activities, but for others it may involve a higher level of support and care.

Asking for help is a positive step forward in maintaining independence and wellbeing. Our focus is to work in partnership with our clients to establish their goals based on their assessed age-related needs and to support them to continue living well and independently.

We provide in‑home and centre‑based respite so carers can take a break from caring responsibilities.

Our Perth‑based dementia consultants offer practical advice, guidance and emotional support for people living with dementia and their families.

Understanding the funding options available

The Australian Government helps older people stay independent by subsidising the cost of home support services. This makes it possible for many of our clients to continue living safely and comfortably in their own homes.

The amount each client may need to contribute depends on the services they receive and, in some cases, their financial circumstances. Co-contributions are determined through a financial assessment by Services Australia.

Under the Support at Home program, funding is provided for specific services based on an aged care assessment. Some clients may also receive quarterly budgets if they were approved for care before the reforms. Any fees or contributions that apply to your situation will be discussed with you before services begin.

Home care funding

Entry-level care

If you need low level support for everyday activities at home, you may be eligible for the Commonwealth Home Support Program (CHSP) through My Aged Care.

Moderate care

The government-funded Support at Home program covers a higher level of care across clinical services (fully funded), independence support, and everyday living.

Restorative care

Short-term care to help you regain confidence and maintain your independence. Services may include physiotherapy, occupational therapy, nurse care and more.

Assistive technology at home

Government-funded equipment and home adjustments to make your home safer and more accessible funded through the Support at Home program.

Flexible options

If you need support right away while waiting for government funding, you can pay privately for a range of services to meet your needs.

Since November 2025, many home care services are now funded through the Australian Government Support at Home program.

Your dedicated Care Coordinator

Based on your aged care assessment, you will have access to a list of approved services. A dedicated Care Coordinator will support you to choose the combination of services that best fits your needs and can be delivered within your budget.

Your Care Coordinator will take the time to understand your goals, preferences, health needs and existing supports. This information is documented in your Care Plan, which is reviewed each year or sooner if your needs change.

Your Care Plan is guided by the Support Plan you receive when you are approved for aged care through My Aged Care. You can adjust your mix of approved services at any time. Your Care Coordinator will help you explore options and make sure the services you receive continue to meet your needs.
