Help at home looks different for everyone and the types of services available to individuals can vary depending on the level of support they have been approved for.

Services can be accessed through the Commonwealth Home Support Programme (CHSP), Home Care Package, Short-Term Restorative Care (STRC) or funded privately. An assessment process determines what program is more suitable.

What are the different types of Home Care Packages?

There are four levels of Government-funded HCPs to meet people’s varying needs for support, with Level 1 packages supporting people with basic care needs through to Level 4, which supports people with high-level care needs.

  • Level 1 basic care needs
  • Level 2 low-level care needs
  • Level 3 intermediate care needs
  • Level 4 high-level care needs

The HCP funding individuals receive is based on their Aged Care Assessment Team (ACAT) assessment, which will determine what level package and services they are eligible for. The higher the level, the more funding the Government provide on the individual’s behalf. If an individual is eligible for funding, a budget will be allocated for them to use for the support and services best suited to their needs and circumstances.

Home Care Packages (HCP)

Home Care Packages (HCP) are available if a person over the age of 65 has been assessed as needing regular support to continue living at home.

Southern Plus offers a range of care and services under HCP including:

– Personal care
– Cleaning
– Social support and companionship
– Shopping
– Allied health support
– Aids and equipment
– General wellbeing
– Clinical care
– In-home respite

Commonwealth Home Support Programme (CHSP)

The Commonwealth Home Support Programme (CHSP) is a Government program that supports older people who are still managing well at home but want some extra assistance. CHSP services are subsidised by the Government, but individuals may be required to contribute to some of the cost.

Southern Plus offers a range of home and community care services under CHSP including:

  • Personal care – help with bathing or showering, dressing, hair care and going to the toilet
  • Cleaning
  • Social support individual – in-home companionship/social visits
  • Social support group – run in a community-based group setting at our respite centres in Forrestfield and Hilton
  • Cottage respite – centre-based

Short–Term Restorative Care (STRC)

The Short-Term Restorative Care (STRC) program supports older people who are experiencing a decline in function/mobility. The program is aimed at supporting individuals to regain their strength and confidence, helping to improve overall wellbeing and quality of life. Our team works with our clients to achieve their goals and boost their ability to complete everyday tasks. Individuals can receive short-term restorative care for periods up to eight weeks, twice in any 12-month period.

The STRC program is available to people aged 65 years and over who:

  • Are feeling run down or have had a recent illness
  • Need help with everyday tasks
  • Have trouble holding or gripping objects
  • Have experienced increased weight loss/weight gain
  • Are not currently receiving a Home Care Package
