Support to age at home

We are committed to supporting our clients to stay in their homes and remain independent for as long as possible.

For some, home care might mean having a bit of extra help with everyday activities, but for others it may involve a higher level of support and care. Either way, it is important to remember that asking for help doesn’t mean loss of independence.

Our focus is to work in partnership with our clients to establish their goals based on their assessed age-related needs and to support them to continue living well and independently.

We support our clients choice to remain living at home by assisting with:

Clinical Care

Our staff can provide clinical care and wound care, including changing dressings, support rehabilitation and developing nutrition plans.

Shopping

Our team can help individuals stay on top of their weekly grocery shop. We can help select, receive and unpacking groceries ordered online. For those who prefer to shop in-store, we can also accompany individuals to the local shopping centre.

Medication Management

Individuals who require a little help managing their medication but do not require a doctor or nurse, can receive support from our team in the privacy of their own.

Domestic Assistance

Our specialist team help our clients keep on top of jobs around the home, from including cleaning and tidying.

Personal Care

Staying healthy and active can be boosted by maintaining a positive appearance. We offer personal care to help our clients start the day feeling positive and stress free. We can help with showering, dressing and hair care.

Allied Health Support

Our allied health specialists help to manage and improve functional mobility and promote personal health with scheduled in-home visits.

Specialised Aids and Equipment

We can provide equipment to empower clients to stay mobile and independent both at home and when they are out and about. Some of these specialised items include wheelchairs, walking frames and bed hoists.

Meals and Nutrition

We can help prepare a range of healthy and nutritional home-cooked meals and can help with shopping to keep the freezer or pantry stocked.

Social Support and Companionship

Living independently in your own home can be isolating for people who don’t have regular visitors. We help our clients to remain socially connected by offering in-home companionship/social visits.

Respite Care

We support carers take a break from their usual care responsibilities by providing in-home and centre-based cottage respite care. In-home respite usually involves a support worker coming to your home so that your carer can go out for a few hours.

Dementia Care

Our dementia consultants can provide Perth-based individuals and their families with practical tools, advice and emotional support.

When Portia’s husband Peter was recommended for home care 12 years ago she signed up at the same time.

Since then her Support Worker Dianne has become more than a carer, she has become a friend.
