What an extraordinary year! We are delighted to share the Annual Report for FY 23/24.
From launching new strategic initiatives, to providing countless activities to help our people connect and new programs to complement our already wide breadth of services, this Annual Report demonstrates our impact and celebrates our accomplishments over the last financial year.

Celebrating our impact
Our 920 employees and 114 volunteers have supported over 3000 West Australians to live extraordinary lives, every day, connected to communities. Here’s a snapshot of our impact.

Easing the transition from hospital to home
We introduced our Community Transition Care Program (CTCP), designed to assist individuals to return home after hospitalisation. Our team of allied health professionals work collaboratively to develop personalised, goal-oriented plans that empower clients to reclaim their independence while reducing the need for long-term care.
Improving our food and dining experience
Good food and enjoyable dining experiences are fundamental to the quality of life for people living in aged care homes.
Over the last year, we have made improvements in response to resident feedback to better cater to preferences and support choice, which has led to an increased food satisfaction score of 90% and reduced weight loss among residents.
Supporting our vitally important people
People who are vitally important is one of our strategic pillars, recognising how important our people are to our success. We have developed a People Strategy that details what our people gain from a career at Southern Cross Care (WA), what we need from them and how we support both elements in our People, Learning & Culture team.
Planning our future services
SCC (WA) provides services in accommodation, aged care and community settings that require activities to be planned to meet the needs of the community and provide the best possible outcomes for residents and clients. To assist us to determine where, when, what and how services should be expanded in the future, we developed a Service Plan and Model of Care (Residential Aged Care) and are now working on a Functional Brief (Residential Aged Care).
Expanding the reach of our mental health services
The Community Mental Health team delivered a record number of support hours, demonstrating both the client’s commitment to their recovery journey and the growing demand for this service. We also supported a growing number of family members and friends of individuals living with mental illness to establish support systems through the Family and Carer Support Program.
Ensuring no one dies alone
SCC (WA) launched the No One Dies Alone program to ensure all aged care residents undergoing palliative care receive companionship and emotional support 24-48 hours before passing. Training was provided to clinical staff and a team of dedicated volunteers and a new app was implemented to support volunteers through the process.
