Support to live independently at home

We are committed to supporting our clients to live independently at home for as long as possible. For some, home care might mean having a bit of extra help with everyday activities, but for others it may involve a higher level of support and care.

Asking for help is a positive step forward in maintaining independence and wellbeing. Our focus is to work in partnership with our clients to establish their goals based on their assessed age-related needs and to support them to continue living well and independently.

We support our clients choice to remain living at home by assisting with:

Clinical care

Our team provides clinical and wound care, including dressing changes, rehabilitation support and guidance on nutrition and recovery.

Shopping

We can help clients manage their grocery shopping, whether that’s selecting, receiving and unpacking online orders or accompanying them to the local shops.

Medication management

For clients who need a little help managing their medication but do not require a doctor or nurse, our team can provide reminders and assistance in the comfort of their home.

Domestic assistance

Our Home Support Team helps clients stay on top of everyday household tasks, including cleaning, tidying and light chores

Personal care

Staying healthy and active can be boosted by maintaining a positive appearance. We can help clients start the day feeling confident and cared for, with support for showering, dressing, grooming and hair care.

Allied health support

Our allied health specialists work with clients to improve mobility, strength and wellbeing through scheduled in-home visits.

Specialised aids and equipment

We can arrange equipment that helps clients stay mobile, independent and safe at home and in the community. This may include wheelchairs, walking frames and bed hoists.

Meals and nutrition

We can assist with meal preparation and planning, helping to create nutritious, home-cooked meals or keep the pantry and freezer stocked.

Social support and companionship

Living independently at home can sometimes feel isolating. We help clients stay connected through regular social visits and meaningful companionship.

Respite care

We support carers to take a break from their usual responsibilities by providing in-home or centre-based respite. In-home respite involves a support worker visiting so carers can take time for themselves.

Dementia care

Our dementia consultants, based in Perth, provide practical advice, emotional support and guidance for people living with dementia and their families.

Assistive Technology and Home Modifications (AT-HM)

Southern Cross Care WA (SCCWA) offers services through the Support at Home’s Assistive Technology and Home Modifications (AT-HM) scheme.

If you’ve been approved for this program through your My Aged Care assessment, you may be eligible to access a wide range of assistive technology and home modifications. These supports are designed to help you stay safe, independent and comfortable in your own home.

Care management

Our Care Management service helps coordinate your supports so you can focus on living well.

A Care Management fee of 10 per cent is deducted from your quarterly budget and held securely by Services Australia.

Care Management includes:

  • Developing and maintaining your Care Plan and budget to ensure they remain up to date and effective
  • Organising and scheduling your services
  • Liaising with your care workers, health professionals and service providers
  • Monitoring quality and safety to maintain high standards of care
  • Connecting you with additional community supports when needed
  • Reviewing and updating your Care Plan as your needs change

Your dedicated Care Coordinator

Based on your aged care assessment, you will have access to a list of approved services. A dedicated Care Coordinator will support you to choose the combination of services that best fits your needs and can be delivered within your budget.

Your Care Coordinator will take the time to understand your goals, preferences, health needs and existing supports. This information is documented in your Care Plan, which is reviewed each year or sooner if your needs change.

Your Care Plan is guided by the Support Plan you receive when you are approved for aged care through My Aged Care. You can adjust your mix of approved services at any time. Your Care Coordinator will help you explore options and make sure the services you receive continue to meet your needs.
