This includes nursing, physiotherapy, occupational therapy, podiatry, nutrition advice, and care management to help you stay safe and maintain your independence.
This includes personal care, social support, transport, carer respite, remedial massage and help with shopping. These supports make it easier to stay connected to your community.
This includes cleaning, laundry, meal preparation, gardening, grocery shopping and home maintenance to help you stay safe and comfortable at home.
To get started, you’ll firstly need an assessment to determine the level of care and services you need. You’ll then follow the steps to access the support you need:
Get in touch with My Aged Care online or call 1800 200 422 to discuss your needs and start your application for home support.
If you are new to home care, the My Aged Care team will organise an assessment to understand your care needs.
You will be notified when your funding is available. You can then select your preferred provider. When you choose Southern Cross Care WA, we’ll get started right away. You can contact our friendly team on 1300 669 189.
Our team will arrange a visit with your Care Coordinator to plan your services and answer any questions you might have.
