In order to be eligible for Southern Cross Care (WA)’s home care services, you will need to apply for a home care package through My Aged Care.
You can contact My Aged Care by calling 1800 200 422 or going to www.myagedcare.gov.au.
To be eligible for a Home Care Package, you must be an older adult who requires various services to help you live independently at home. Your eligibility will be confirmed through an ACAT assessment, identifying whether you have high or low-level care needs.
Your financial situation won’t impact whether you’re eligible, but it may be assessed to determine the amount you might need to contribute.
If you are assessed as needing lower-level assistance, a different program called the Commonwealth Home Support Programme or Short-Term Restorative Care could be a better fit.
