Dementia Care Services
A very special service.
We offer specialised dementia care services for people with moderate to severe dementia.
Your GP will refer you to an Aged Care Assessment Team. The Dementia Extended Aged Care at Home program provides up to three visits a day, plus access to our respite services at Success or Vincent House. We also run two dementia-specific day respite services, where you're welcome to spend a comfortable, enjoyable day in the company of other adults.

Rest and respite at Success House
Dementia specific respite is provided Monday to Saturday. Overnight respite is available Monday to Friday.
We offer this service to people living in the cities of Fremantle, Cockburn, Melville and the Town of East Fremantle.

Dementia-specific respite.
Beautiful period home in the shade of a jacaranda, right in the heart of North Perth.
A perfect place for a day's break for you and your primary carer. This is home away from home, and clients take part in everyday activities to improve life skills and levels of independence. They'll prepare morning tea, make fresh pasta, water gardens, head to a nearby café. It's a stimulating environment that focuses on the individual.
Open Monday to Saturdayay, 8am till late.
Mondays and Fridays are Italian respite days with Italian-speaking support staff.
Overnight respite is available Monday to Friday. Transport is provided.
Service available to people Living in the cities of Perth, Stirling, Bayswater and Swan, Towns of Vincent and Bassendean, and Shires of Mundaring and Kalamunda.
